We offer you:
• Open 5 ½  Days a week
Chronic illness treatment; some examples for chronic illnesses which we manage is hypertension, diabetes, hypercholestermia, COPD and asthma.
• Organising your management plan and team care plan.
• Skin Cancer Check/Minor Skin cancer surgery
• Pathology collection centre.
• Immunisations (children and adults and children health assessment)

• ECG: heart check

• Minor surgery: stitching cuts,
removing moles, skin cancers

Information

Fees:
We are a bulk billing practice for all Children under 16, Pension and Health Care Card Holders, and follow up visits.
If you do not hold a valid Medicare Card the consultation fee will be $69.00
Pre-employment Medicals and Driving Licence Medicals: These are not covered by Medicare and will incur a cost.

Cancelation Policy: Due to having one Doctor, all appointment slots are extremely important.
Therefore a fee of $32.00  will be charged for non-cancelation of appointments 2 hours prior to appointment. We may be able to offer your appointment to another patient.

OUR PRACTICE
Our friendly Administration Team consists of:  Cathie Bickley, our Practice Manager and our Medical Receptionist, Mel Brown.


Management of Personal Health Information
The practice has strict guidelines on confidentiality, patient consent, consent for a 3rd party to obtain information, transferring of health information and access and security of personal health information.  It adheres to the the National Privacy Principles Act and the Health Records and Information Act.

URGENT RESULTS
Your Doctor or the Receptionist will attempt to contact you immediately via contact details supplied. If our attempts at contact are unsuccessful, a letter will be forwarded to your address to contact us urgently for an appointment.

NON URGENT RESULTS
The Receptionist will contact you via phone to advise that you need to make an appointment to see your Doctor. If our attempts to contact you are unsuccessful, a letter will be forwarded to your address to contact us for an appointment.

If your results are NORMAL we will Not contact you. You are welcome to phone us to check if your results have been returned to us, but you will have to make an appointment to discuss these results.
At no time are results provided to you over the phone due to confidentiality laws.

Patients are contacted for recalls and reminders. If our attempts at contact are unsuccessful a letter will be forwarded to your address to contact us for an appointment.

Confidential information is not included in these letters). All personal information is stored on a program specific, encrypted computer data base

CONTINUOUS EDUCATION
The practice is committed to supporting the doctor, nurses and administration staff to participate in on-going education.

If you need more time with the doctor, please tell the receptionist when you make your booking. Urgent medical problems will always be dealt with promptly.

You can contact your doctor by ringing during surgery hours. Emergency calls will always be put straight through.

When the surgery is closed, please ring Health Direct on 1800 022 222.  Alternatively, you can attend the Emergency Department at Kalgoorlie Hospital.
